Tragic German Bight Collision Ruled 'Wholly Avoidable'

By MGN Editorial•February 13, 2026 at 03:24 AM

Investigation finds basic safety rules were ignored, leading to a preventable tragedy that claimed five lives.

In a sobering report, the UK's Marine Accident Investigation Branch (MAIB) has concluded that the 2025 collision between the cargo ships Verity and Fortuna in the German Bight was a 'wholly avoidable' tragedy. According to the MAIB's final investigation, the incident that claimed the lives of five sailors could have been prevented if the crews and shore-side authorities had followed established collision avoidance protocols. The MAIB found that the Verity and Fortuna were on a direct collision course, but the crews of both vessels 'delayed taking decisive action' to avoid the crash. Even worse, the report states that 'shore-side intervention came too late to stop the inevitable'. Ultimately, the MAIB determined that 'basic collision regulations were ignored' in the moments leading up to the fatal accident. 'This was a completely preventable incident,' said MAIB Chief Inspector Sarah Treseder. 'The evidence shows that if the crews had followed standard safety procedures and the shore-side authorities had acted more quickly, this terrible loss of life could have been avoided.' The MAIB's findings underscore the importance of strict adherence to maritime safety protocols, effective communication between vessels, and timely intervention from shore-based traffic management systems. This tragic case serves as a sobering reminder to the industry that complacency can have devastating consequences. As the maritime sector continues to evolve, stakeholders must remain vigilant in upholding the highest standards of safety and seamanship to prevent such heartbreaking incidents from occurring in the future.
